The Application Form with Prospectus can be obtained (i) in person from Academy for Medical Education & Research, Vasai Road(W) on payment of application fee of Rs.250/- either by cash or (ii) by post, sending a requisition to The Director, Academy for Medical Education & Research, Vasai Road(W) along with a Demand Draft for Rs.290/- drawn in favour of The Director, Academy for Medical Education & Research, payable at Vasai Road(W) specifying the programme.

    The Application Form is not transferable and the application for admission should be in the prescribed form of the academic year  2009-2010.
